Q. How do I make an appointment?
A. Our office hours are 9:00 am to 5:00 pm, Monday through Friday. We have the utmost respect for everyone’s time, and we do everything we can to make the best use of both your time and our physician’s time.

Please call (415) 202-0250 to request an appointment. When you call, the scheduler will ask for the following information:
Your name
Your date of birth
Brief description of the reason for the appointment
Your Address
Home and alternate (work) phone numbers
Your primary physician or health care provider, and whether or not you would like us to send a report of your child’s visit to that provider
IN CASE OF EMERGENCY
The phones are always manned day and night either by the office or the answering service. If you ever feel that you are experiencing a medical emergency please call 911. If the problem is urgent but not an emergency please call (415) 202-0250 and the doctor who is on call will be notified. A doctor will either return your call or you will be instructed to go to the emergency room at the hospital where the doctor will meet you.

Q. How do I obtain a refill of my prescriptions?
A. We prefer that you ask for prescription refills during your normal visit. However, if you need a prescription refill prior to a visit, please call our prescription line at (415) 202-0250. You will be prompted for the following information:
Your name
A daytime telephone number with area code
The physician who prescribes your medication
CPI or registration number
Your birth date
The name and spelling of the medication
The approximate number of doses you have left
The Pharmacy name and phone number with area code

It is helpful to have the actual prescription bottle handy when you call our prescription line. If you have only a few pills remaining, please indicate that the request is urgent when asked how many doses you have left. It is much better to contact our office before the situation is urgent to give ample time to consult with your physician if necessary. Please allow 1 to 3 days for routine prescription refills and 24 hours for urgent requests. You will receive a confirmation phone call when your prescription has been filled or called to your pharmacy.

Q. What are your office hours?
A. Our office hours are 9:00 a.m. to 5:00 p.m., Monday through Friday. Be assured that we provide emergency coverage 24 hours per day, 7 days a week.

Q. What do I need to bring on my first visit?
A. On your initial visit, please bring the following items:
Identification
Insurance card(s)
Written referral form, if required
List of medications you are taking
Pertinent test results (lab and x-rays results.)

Also, be prepared to share your medical history with our physicians. This will include any medical conditions you have or may have had in the past, surgical history, any medications (prescription or over-the-counter) you are currently taking (include name, dosage and frequency), and any allergies you have to medications or foods.

Q. Insurance
A. Referrals, Pre-Authorization & Second Opinions
Many insurance companies require pre-authorization on many procedures. It is important for patients to know the requirements of their insurance. Second opinions also may be needed before procedures are performed. If your plan requires a referral from your primary care physician it is your responsibility to obtain this prior to each visit.

Billing Procedures
We will bill most insurance companies as a courtesy to our patients. We are under contract with many HMOs and PPOs.

It is important you understand that if you have health insurance, this is an agreement between you and your insurance company. Your doctor's bill is an agreement between you and the doctor regardless of your insurance. We ask that payment be made at time of service unless other arrangements have been made. We accept VISA and MasterCard.

We do bill Medicare and accept assignment from them.
